Individual Program Applications

Application Fees Graduated Fee Schedule
An application fee is required for each educational activity. This fee is non-refundable. The fee is determined by the number of Contact Hours (See section L. of the application) for which you apply and the date the completed application is received by AaNA.

*Late Fees:
- 6-8 weeks prior to presentation — Add $25
- 4-6 weeks prior to presentation &8212; Add $50

* The late fees that are listed on the application apply to applications received by AaNA less than eight weeks prior to the presentation date. It does not "accelerate" the review process.
** Not available to presentations over 18 hours
Please note that it is possible late applications may not complete the process prior to the presentation date!
Provider Applications
The Provider Application form is available in PDF format.

AaNA recommends sending your application in at least 60 days prior to expiration of your provider accreditation.
Applications after February 2003 will be good for 3 years (previously 2).
AaNA plans to make the new application available on disc by February 1, 2003. Please call for a copy.
Prices:
- Providers offering less than 30 CE offerings per year &8212; $1,725
- Providers offering between 30 - 75 CE offerings per year &8212; $2,500
- Providers offering over 75 CE offerings per year &8212; $3,100

PLEASE NOTE:
ALL applications must be submitted at least 21 days prior to the CE offering and the contact person must be available to work with Ms. Monsen to address deficiencies, in order to ensure approval. CE applications submitted less than 21 days from the offering may not be accepted or may not be approved. There is NO RETROACTIVE approval for CEs &8212; Approval must be complete prior to the beginning of the program.
Tips for Downloading the Forms to Your Computer
We do not have the capability to process your application on-line. But if you follow these instructions, you will be able to prepare it on your computer and then mail it to our office via US Postal Service.
This process works for Microsoft Internet Explorer and Microsoft Word. If you use other software, the process may be slightly different.
- Under edit, select all.
- Copy, then shrink your internet access.
- Open a new word processing document. See note below.
- Paste and save.
Note: This document is formatted for Microsoft Word 97/2000. It can be converted to other word processing software, but the formatting may be altered.
Common Problems to Avoid
Most people have difficulty writing a "behavioral objective." The purpose of a behavioral objective is to allow the course participant to measure an objective to see if it has been achieved. A behavioral verb is used to make an objective measurable, such as: describe, list, discuss, and define. A partial list of these verbs are listed on the application and are available for download from this site (MS Word format).
